By signing this mandate form, you authorise the Creditor to send instructions to your bank to debit your account, and your bank to debit your account in accordance with the Creditor's instructions.
You have the right to be reimbursed by your bank in accordance with the conditions set out in the agreement you have signed with it.
A request for reimbursement must be submitted within 8 weeks of the date your account is debited for an authorised direct debit.
